Why Scans Don’t Always Match Pain

If you’ve ever had an MRI, X-ray, or CT scan that didn’t seem to match the pain you’re feeling, you’re far from alone. It’s one of the most common frustrations people bring to physiotherapists and health professionals:

“My scan looks normal — yet I still hurt.”
“My report looks terrible — but I barely feel anything.”

This mismatch between imaging and experience is real — and it’s not because your pain isn’t real. It’s because scans show structure, not pain. Pain is far more complex than what appears on a screen.

What Scans Are Designed to Show — And What They Aren’t

Imaging tools like MRIs, X-rays, and CT scans give us a detailed picture of your anatomy — bones, discs, joints, ligaments, and other tissues. They’re incredibly valuable for:

– Ruling out fractures

– Identifying major tears

– Spotting tumors or serious pathologies

– Guiding surgical planning

But here’s the key point:
Imaging shows structure, not pain. These tools don’t reveal how your nervous system is interpreting signals, how your muscles are functioning, or how your body moves.

Why the Scan–Pain Gap Happens

1. Structural Changes Can Be Normal

Many “abnormal” findings are actually common age-related changes. For example:

– Disc bulges

– Degeneration

 – Joint wear

Studies show that a large percentage of adults with no pain at all still have these findings on MRI. These changes are often similar to gray hair — common and part of aging — not necessarily pain generators.

Scans are extremely important when there are red flag symptoms, such as:

– Progressive weakness

– Loss of bladder or bowel control

– History of cancer

– Severe trauma

– Signs of infection

In these cases, imaging plays a crucial role in ruling in or out serious disease and guiding urgent care.

But for most common musculoskeletal complaints — like back pain, neck stiffness, or joint discomfort — scans are just one piece of the puzzle.

What Matters More Than a Scan

Instead of relying only on imaging, a comprehensive approach looks at:

– Movement patterns — how you bend, lift, walk, or reach

– Strength and control — which muscles are strong or weak

– Pain history — what makes pain worse or better

– Functional assessment — how your body works in daily life

When clinicians focus on these areas — not just what an image looks like — they uncover real causes of pain and design better recovery plans.
